When faced with a new diagnosis of Diabetes, Hypertension, or Depression we know the information and education shared with you can be overwhelming. We also know that after you leave the clinic you may have questions that come up about monitoring and treating your condition. We would like to know if you would appreciate a contact from a nurse to see how you are doing, and answer additional questions you may have.
